Notice of Infringement – Claim
- A signature, either physical or electronic, from the copyright owner (or an individual authorized to represent the owner);
- An identification of the copyrighted work that is claimed to have been infringed;
- Identification of the infringing material that is requested for removal, including adequate information to help the service provider locate the material. [Please include the URL of the page in question to aid us in identifying the allegedly infringing work];
- Details that are sufficient for the service provider to contact the complainant, which must include your name, physical address, email address, phone number, and fax number;
- A declaration that the complaining party genuinely believes that the material’s use is unauthorized by the copyright agent; and
- A confirmation that the details in the notification are accurate, and, under the penalty of perjury, that the complaining party is authorized to act on behalf of the copyright owner.
Title 17 USC §512(f) outlines civil damage penalties against any individual who knowingly and materially misrepresents certain information in a notification of infringement as per 17 USC §512(c)(3), including costs and attorney fees.

Be aware that we may disclose the identity and information pertaining to any copyright infringement claim we receive to the party alleged to be infringing. By submitting a claim, you acknowledge and agree that your identity and claim may be communicated to the alleged infringer.
Counter Notification – Restoration of Material
Should you receive a notice regarding the takedown of material due to a copyright infringement claim, you have the right to submit a counter notification in an attempt to restore the material in question to the site. This notification must be submitted in writing to our DMCA Agent and should include the following elements as outlined by 17 USC Section 512(g)(3):
- Your signature, whether physical or electronic.
- A description of the material that was removed, along with its original location prior to the takedown.
- A statement under penalty of perjury asserting that you have a good faith belief that the material was taken down due to a mistake or misidentification.
- Your name, address, and telephone number, along with a statement consenting to the jurisdiction of the federal district court for the judicial district where your address is located (or, if outside the United States, that you consent to jurisdiction of any judicial district where the service provider may be found), affirming that you will accept service of process from the individual or entity that issued the original infringement notification.

Repeat Infringer Policy
We take copyright infringement seriously. In line with the requirements of the Digital Millennium Copyright Act concerning repeat infringers, we keep a record of DMCA notices from copyright holders and make a diligent effort to identify any repeat infringers. Those who breach our internal repeat infringer policy will face account termination.
